Title: Client care analysis of assessment

The following leaning outcomes must be covered and indicate where each is covered.
1. Discuss the contribution of nursing knowledge to patient care
2. Demonstrate an understanding of the evidence base for the care identified on a patient s care plan
3. Identify how physiological, psychological, and sociological needs , may alter on admission to hospital
4. Asses, plan and deliver care for the hospitalised patient
5. Reflect on own inter-professional communication skills, working in partnership with patients and carers and other agencies.

The focus of this assignment is on the assessment and care planning of an elderly client (patient) you have nursed and cared for who has chronic obstructive pulmonary disease (COPD) in an hospital where you worked as a student nurse
Selection of the patient: A case study
You should select a patient in whose care you will have made a significant and direct contribution over an identified period of time. For instance in an hospital where you have worked as a student nurse ( during placement) ; and you are strongly advised to discuss why you chose that particular patient.
The assignment requires you to:
” Make an holistic assessment of your patient s needs/problem and analyse the assessment process

” From the assessment process identify two of the patients needs and discuss these in relation to the care given, with reference to psychological (e.g anxiety due to hospitalisation, loneliness, separation, isolation), social (Socio-political NHS Plan, making a difference, modernisation agency, waiting lists, choice, day surgery. Health promotion), biological and very importantly related the patho-physiology.

” Reflect on own inter professional skills in relation to the care planning and delivery process (that is, demonstrate inter-professional awareness within your exploration of this scenario, identify at least 4 non nursing professions/agencies that may be involved and discuss the role of these professions/agencies and the channels of communication between all who are involved. There should also be some appraisal of the way in which team work and collaboration may affect the outcome of the events described and the issues raised in the scenario.

” Throughout the discussion review and evaluate the literature and the evidence base that contributes to care planning and delivery

This patient has been admitted on to an acute ward. What issues would you need to consider to ensure a comprehensive assessment can be obtained from the patient in the scenario?
