Carefully read chapter 18 of Omachonu and Ross (2004) and ensure that you understand the meaning of a Quality Improvement (QI) story. A quality improvement story covers seven steps.

More specifically, a QI Story is a step-by-step guide for problem solving or process improvement.
(2) Address the following items/exercises (a – f) shown below and record your comprehensive answers on a word document for future reference. Please do not submit your answers to Blackboard for items a – f shown below. The answers should be retained by you and referred to as you address criterion 3 shown below.
a. What is a QI story?
b. Discuss several reasons for using a QI story.
c. List the basic steps of a QI story.
d. Identify one of the best ways to demonstrate the importance of a problem to management.
e. What are the two types of action that can be taken? Give an example of each action type.
f. What are the basic requirements of a QI story?
(3) Identify a former quality problem that has been solved. For this criterion (#3) you may identify a quality problem that has been solved and identified as such among the current literature. You may also identify a quality problem that has been solved within your own work environment or another work environment of your choosing. Further, you may also work with ONE other student in this class; and you may consult someone currently working in industry as well to assist you as you develop your own version of a QI Story. You do not need to contact this instructor for permission to work with someone on this assignment. CAUTION: each student must write his/her own final paper when presenting the seven-step QI Story of a former quality problem. This means that two-person student teams may NOT submit duplicate QI Stories for this assignment! Each student must write his or her own version of a quality improvement process (or past process) in their QI Story.
As you refer to the seven steps (see page 320) you will need to carefully document your answers and/or findings of how the problem was solved in a detailed QI Story format. Please refer to figure 18-1 on page 320 to elevate your understanding and presentation of your QI Story. The figure shows the typical relationship between the QI process and the Plan, Do, Check, Act (PDCA) model. Pages 320-325 offers details on the seven basic steps you will use to explain how the problem was solved.
(4) Your paper should include a cover page, abstract of your report, and address each of the seven steps (listed on page 320) and incorporate these into an organized QI Story framework.
(5) Undergraduates should submit a minimum of six pages, single or double-spaced (12 point font), including the cover page and abstract page.
(6) You may include charts, graphs, or figures to better illustrate processes or outcomes within your report if you so choose. Please cite all sources, including any personal communications from those you may elect to consult within industry. A sample reference for personal communications is shown below.
• Please note: For interviews, email, and other personal communication do not include these in the Reference list. Instead, parenthetically cite the communicator’s name, the phrase “personal communication,” and the date of the communication in your main text only as shown: (E. Robbins, personal communication, January 4, 2001). Again, this should be in the text immediately following the personal communication.
• Please note the example of a personal communication cited within a QI Story document:
The TQM officer for XYZ Pharmaceuticals informed this student of steps and tasks they utilized to identify the cause for the problem, including the initial tracking indicator their organization used to ensure reliability (B. Smith, personal communication, June 22, 2011).
